목적: 최근 임상적으로 급성 이명에 대해 steroid 치료효과에 대한 연구가 보고되고 있다. 하지만 이에 대한 기초 연구는 부족한 실정이다. 본 연구는 이명 모델에서 전신적인 dexamethasone treatment와 intratympanic dexamethasone 시횅시 cochlear blood flow개선효과와 Gap prepulse inhibition of startle reflex (GPIAS) 개선효과를 비교해보고자 하였다. 방법:S-D rat 14마리를 대상으로 무작위로 두 group으로 나누어, saliyate 복강 투여후 이명모델을 GPIAS를 사용하여 확인한 후 전신적으로 투여한 intraperitoneal dexamethasone injection group, intratympanic dexamethasone injection group으로 나누어, cochlear blood flow, GPIAS 시행하여 비교하였다. 결과:Salicylate 복강 투여후 cochlera blood flow 는 time dependent manner로 감소하였으며, dexamethasone 투여는 salicylate로 감소된 cochlear blood flow를 time-dependent manner로 증가시키는 효과를 보였으며, intratympanic dexamethasone injection group이 intraperitoneal dexamethasone injection group 보다도 cochlear blood flow가 의미있게 증가되는 효과를 보였으며 GPIAS 또한 intratympanic dexamethasone injection group이 intraperitoneal injection group보다 더 개선되는 효과를 보였다. 결론:Intratympanic dexamethasone injection은 steroid 전신적인 치료로 유발되는 부작용을 피할 수 있고 cocochlear blood flow , GPIAS 개선효과가 전신투여보다 더 좋아서 steroid 급성이명에서 이명 치료에 도움이 될것으로 사료된다.
